Enter for a Chance to Win $100.00!

Would you like to have a chance to win a $100.00 check from TLC Pediatrics just for having a well child check-up or sports physical?  TLC Pediatrics will enter the names of all children who complete their well-child checkup or sports physical at TLC Pediatrics, during the month of July 2023, into a drawing for a chance to win one of four $100.00 checks from us.  Call us to see if your child is eligible for a well child check-up or sports physical during the month of July 2023!  The names will be chosen randomly during the first week of August 2023 and the winners notified. 

Details for the drawing are included below.

1. One entry for each child who is eligible (per their insurance) and has a well-child check-up or sports physical completed in July 2023 at TLC Pediatrics.
2. Please stop by the front desk after your child’s check-up to confirm that their entry has been made.  If your child has already had a well-child check-up or sports physical in July 2023 at our clinic, they will be automatically entered.  You may decline the entry if you prefer.
3. Four names will be chosen randomly during the first week of August 2023 to receive a check.
4. A $100.00 check will be written to the guarantor of the child’s account.
5. If the winner has an outstanding balance due at the clinic for any of their children, the $100.00 prize will be applied to the balance and a check for the remainder of the prize (if any) will be written.
6. The winner must pick up the check in person at TLC Pediatrics, show their ID, and sign for the check.
7. Children of employees of TLC Pediatrics are not eligible for entry into the drawing.
8. We will contact you if you are a winner.
9. We will post a list of the winners’ initials only due to privacy reasons.
10. TLC Pediatrics reserves the right to change any of the details and rules at any time.
